Back on Track is not a counseling practice. We work alongside families, schools, behavioral health providers, employers, and community organizations to ensure individuals receive coordinated support and consistent guidance throughout their journey. Our role is to bridge systems, remove barriers, and help participants stay connected to the resources they need to succeed.
Connect: Mentorship & Relationship Building
Connect provides positive, supportive relationships that help participants build confidence, accountability, leadership, and personal purpose.

Navigate: Life Skills & Success Training
Navigate equips participants with practical tools needed to successfully navigate school, work, relationships, and everyday life.

Launch/Rise: Youth Support Program
Designed for students in grades 2–12, Launch/Rise focuses on behavioral stability, academic engagement, and personal development.
Services include:
- After-school programming

- Character development
- Family engagement

Return: School Transition Services
Return supports students transitioning back to traditional educational settings following suspension, alternative placement, or significant school disruption.
Support includes:
- School transition planning
- Home support coordination
- School collaboration
- Accountability monitoring
- Family engagement
Restore: Reentry & Reintegration Support
Restore helps justice-involved individuals build stability, strengthen accountability, and successfully reintegrate into their communities.
Services include:
- Workforce preparation
- Resource connection
- Community support
- Goal planning
- Reentry coordination
