Our program is a comprehensive initiative aimed at supporting at-risk pre-teens, teens, and adults by providing tailored services to help them avoid or exit the legal system.

Mission

Back on Track empowers individuals and families by providing compassionate, community-based support through mentorship, advocacy, life skills development, and coordinated services. Through culturally responsive guidance and meaningful connections, we help people overcome barriers, build stability, and move toward lasting personal growth and success.

Vision

Back on Track envisions communities where individuals and families are supported, connected, and equipped with the resources, relationships, and opportunities needed to thrive, grow, and create positive futures for themselves and those around them.

Key Components of the Program

  • Individualized Support Plans: Engaging with clients, their families, and relevant community members to develop personalized plans that equip individuals with the necessary tools for success.
  • Collaborative Efforts: Maintaining communication with probation officers, law enforcement, educators, and other stakeholders to continuously refine and enhance support strategies.
  • Therapeutic Services: Individual Therapy, Family Therapy, Group Therapy, Peer Support Groups, Mentorship Programs and Life Coaching.

These services are designed to address the diverse needs of participants, fostering personal growth and reducing recidivism.

Program Goals:

  • Prevent Legal System Involvement: Provide resources and support to deter individuals from engaging in activities that could lead to legal consequences.
  • Facilitate Successful Re-entry: Assist those exiting the legal system in reintegrating into society by offering continuous support and access to necessary services.
  • Promote Community Collaboration: Work closely with community members and organizations to create a supportive network that encourages positive outcomes for participants.

This program aligns with national efforts to support at-risk youth and adults through comprehensive, community-based services. For instance, the Office of Juvenile Justice and Delinquency Prevention (OJJDP) supports mentoring services for young people at risk of delinquency and juvenile justice system involvement.
